Utah Tax Form TC-40 - Individual Income Tax Return

Utah Tax Form TC-40 is required by all Utah residents who fall under the following categories:

1. Every Utah resident who is required to file a federal income tax return;
2. Every non or part-year resident having income from Utah sources who is required to fi le a federal return;
3. Taxpayers requesting a refund of any overpayment of income tax regardless of the amount of gross income; or
4. Every individual with Utah income that meets the income requirements (see PDF instructions for details)
