If you are claiming credit for taxes paid to another state or country (Form 309), Form AZ-8453 must be mailed to the Arizona Department of Revenue within one (1) working day AFTER receiving the acknowledgment that the state return was accepted. Form AZ-8453, the required attachments to Form 309, and if an amount is entered on line 3 of Form AZ-8453, state copies of federal Forms W-2, W2G or 1099 are required to be sent to the department and must be mailed to: Arizona Department of Revenue PO Box 29237 Phoenix, AZ 85038-9237
The AZ-8453 should not be mailed to the department unless the department requests the document, or you file Form 309.
State AZ Amount Owe Payment Option
You have the following options to pay your tax:
  1. E-Check: 

    Use the Make a Payment link on the homepage of https://www.AZTaxes.gov. Note: if you make a timely estimated or extension payment online, you do not need to file a form.

    Registration is not required for persons who want to make Individual Income tax payments for their Estimated Tax Payment, Payment of tax for E-Filed returns, Extension Payment and Tax Liability Payment.

    When using the Make a Payment option on AZTaxes.gov, you will be given an option to pay by e-check.

    Note: For correct application of your payment you must be sure to use the correct routing and transit and bank account number for your payment. If any part of these numbers is incorrect or missing, your payment will be returned and you will be assessed a Return Check Fee for the transaction.

  2. Installment Payments: To make payment arrangements, please contact our Collections section at 602-542-5551. In Tucson, call 520-628-6442. Any Revenue Officer can assist you.

  3. Credit Card: Individual Income Tax users can use the Make a Payment link on the homepage of https://www.AZTaxes.gov. Then select the Credit Card option when making an electronic payment. You can pay your taxes with an American Express, Discover, Visa, or MasterCard credit card, or with a Visa or MasterCard branded debit card.

    Direct Debit is supported. If paper payment is required for any reason, it is necessary to pay any amounts owed by April 17, 2006 to avoid penalty and interest. Payment together with the AZ-140V is to be mailed to: Arizona Department of Revenue AZ-140V PO Box 29237 Phoenix, Arizona 85038-9237

  4. Mail in a Check.

You have the following options:
  1. Apply your refund amount to next year's tax payment.
    On 140, 140PY and 140NR 
  2. Contribute your refund amount to charity.
    Education, Child Abuse, Olympics, Wildlife, Domestic Violence, Political Gift, Clean Elections, Neighbor Helping Neighbo 
  3. Receive your refund by check through mail.
  4. Receive your refund by direct deposit to your bank account.
